The fifth batch of state-organized centralized drug collection results announced an average price reduction of 56%
Recently, the results of the fifth batch of centralized drug procurement organized by the state were officially announced, and 61 drugs were successfully purchased, with an average price reduction of 56%. These drugs with reduced prices include drugs for common diseases and chronic diseases such as hypertension, coronary heart disease and diabetes, as well as drugs for major diseases such as lung cancer, breast cancer and colorectal cancer. In October this year, patients are expected to use this batch of centralized drugs with reduced prices, and the burden of medical treatment will be further reduced.
High-value medical consumables also began to be purchased in centralized quantities, and cardiac stents became the first variety to be collected, and the average price dropped from 13,000 yuan to around 700 yuan. At the same time, the medical insurance catalogue is adjusted every year, and some expensive new drugs and life-saving drugs enter the medical insurance through medical insurance negotiations. These measures reduce the burden of drug use by hundreds of billions of yuan for the people every year, and at the same time instigate the reform of "three medical linkage", which greatly enhances the people’s sense of acquisition, happiness and security.
Centralized drug collection has reduced expenditure by 150 billion yuan.
Uncle Hong, a 76-year-old from Xiamen City, Fujian Province, has been suffering from diabetes for more than 20 years and hypertension at the same time. He started taking Betangping more than 10 years ago. He needs 3 boxes a month, each box costs more than 90 yuan, and it costs 3,300 yuan a year. In August last year, when Uncle Hong went to the hospital to prescribe medicine, he found that the price of medicine was cheap, only 5.42 yuan for a box. "I couldn’t believe it at the time, so cheap. Thanks to the good policies of the country, it has saved money for our diabetic patients. " Uncle Hong said.
Baitangping is a commonly used hypoglycemic drug, but its price is high, so diabetic patients need to take it for a long time, and the cost burden is heavy. In January last year, the second batch of drugs organized by the state successfully purchased the drug, which greatly reduced the drug burden of diabetic patients.
Like Bai Tangping, there are many drugs that benefit the people through centralized collection, such as gefitinib tablets for the treatment of non-small cell lung cancer and oxaliplatin injection for the treatment of colon cancer. Many large varieties of clinical drugs have achieved significant price reductions, such as metformin, budesonide inhalation for bronchial asthma, entecavir tablets for chronic hepatitis B, and ceftazidime injection for anti-infection drugs. The sharp price reduction of these drugs has benefited a large number of patients, especially those with chronic diseases and tumors.
Since December 2018, the state organized "4+7" 11 cities to carry out centralized drug procurement, China has organized five batches of centralized drug procurement, and a total of 218 drugs have been successfully purchased. A large number of drugs have reduced their prices through centralized procurement, with an average decline of more than 50%, and the accumulated drug cost savings have exceeded 150 billion yuan. Recently, the fifth batch of centralized drug purchase has set a record in the quantity and amount of drugs purchased, and the number of foreign-funded enterprises selected is also the highest in the national drug purchase.
Wu Ming, assistant director of peking university health science center, said that from the current situation, centralized drug purchase with quantity is a breakthrough in the reform of compensation mechanism in public hospitals and the promotion of "three-medical linkage". With the expansion of centralized procurement coverage, the continuous improvement of rules and the coordinated efforts of supporting policies, the effect of purchasing with quantity will become more and more prominent, and the four effects at the beginning of policy design will be gradually realized, namely, drug price reduction and quality assurance, transformation and upgrading of pharmaceutical industry, deepening reform of public hospitals, and reducing burdens and increasing efficiency of medical security.
The price of high-value medical consumables has dropped by more than 90%
At 1: 37 am on January 1, 2021, 76-year-old Ms. Xiang entered the catheter room of the chest pain center of the First People’s Hospital of Xiangtan City, Hunan Province for 4 hours because of chest pain, and performed emergency coronary angiography, intracoronary thrombolysis and stent implantation, using the national centralized procurement of cardiac stents with a price of 798 yuan, becoming the first patient in Xiangtan City to enjoy the benefits of centralized procurement of cardiac stents.
Coronary stent, commonly known as heart stent, is a kind of high-value medical consumables, which is used for interventional therapy of acute coronary heart disease. Because coronary heart disease has become a common disease in China, acute myocardial infarction is more and more frequent. Although the price of cardiac stent is high, the number of clinical use is increasing year by year. On November 5, 2020, the state organized the first batch of high-value medical consumables to be purchased centrally, and the heart stent became the first variety to be collected. After centralized purchasing, 10 products were selected, and the average price dropped from 13,000 yuan to around 700 yuan, with a drop of more than 90%. One drug-coated stent system (rapamycin), which has just been on the market for three years, reported the lowest price of 469 yuan, a decrease of 96%.
This collection of cardiac stents is expected to save 11.7 billion yuan. After the collection of cardiac stents, the collection of coronary dilation balloons has been carried out in various places, and the overall cost of cardiac stent implantation has been further reduced. In addition, the medical insurance reimbursement will further reduce the patient’s cost burden.
In the future, more high-value medical consumables will be brought into centralized procurement, benefiting more patients. In June this year, the National Medical Insurance Bureau and other eight departments issued the Guiding Opinions on Organizing the Centralized Procurement and Use of High-value Medical Consumables by the State, which clearly pointed out that it is necessary to organize the centralized procurement of high-value medical consumables by the State, explore and improve the centralized procurement policy, and gradually expand the coverage. Recently, the Office of Joint Procurement of High-value Medical Consumables organized by the State issued the Announcement of Centralized Procurement of Artificial Joints (No.1), which carried out centralized procurement for the initial total hip replacement and the initial total knee replacement.
The centralized procurement of drugs and high-value medical consumables has greatly reduced the burden of patients’ medical expenses. And the benefits brought by centralized purchasing are not limited to these. The state organizes and coordinates the joint efforts of medical insurance, health, drug supervision and other departments to ensure that the selected price in centralized procurement is implemented and that the people can really use high-quality and high-priced products.
Negotiate access and reduce the prices of innovative drugs and patented drugs.
"Can you drop another 4 cents?" In 2019, a video of the national medical insurance negotiation scene was on fire. The drug mentioned in the video was Dapagliflozin tablet (Andatang) for treating type 2 diabetes, which was finally reduced from 5.62 yuan in the first quotation to 4.36 yuan.
Such a situation can be found everywhere in the national medical insurance negotiations. A large number of big-name new drugs and expensive drugs have been reduced to the lowest price in the world through negotiations, such as 100 mg /10 ml of cindilizumab injection (daboshu), an immune drug for treating tumors. After medical insurance negotiations, the price has dropped from 7838 yuan to 2843 yuan, a drop of more than 60%; The price reduction of imported hepatitis C drugs exceeded 80%, which was the lowest in the world.
Since the establishment of the National Medical Insurance Bureau in 2018, China has conducted three medical insurance catalogue access negotiations. In 2018, special negotiations on anticancer drugs were held, and 17 drugs were successfully negotiated, with an average decline of 56.7%, covering more than 10 kinds of cancer treatment drugs such as non-small cell lung cancer, chronic myeloid leukemia, childhood acute lymphoblastic leukemia and melanoma; In 2019, a total of 97 drugs were successfully negotiated, of which 70 new drugs were negotiated with an average price drop of 60.7%, and 27 renewed drugs were negotiated with an average price drop of 26.4%. Related drugs involve more than 10 clinical treatment fields such as cancer, rare diseases, hepatitis, diabetes, cardiovascular and cerebrovascular diseases; In 2020, 119 kinds of drugs were successfully negotiated, with an average price reduction of 50.6%. The payment standards for drugs were determined simultaneously, and the patient’s expenses were reduced by nearly 70%.
The drugs negotiated by medical insurance are exclusive drugs, often innovative drugs and patented drugs, with high prices. Medical insurance plays a strategic purchasing advantage and talks about reasonable prices in the world’s largest market with 1.36 billion insured people, greatly reducing the burden on patients. According to statistics, compared with before the negotiation, through the dual effects of negotiation and medical insurance reimbursement, in 2020, the cumulative patient expenditure will be reduced by over 80 billion yuan.
In order to ensure that patients can buy, use and reimburse the negotiated drugs in the first time, in May this year, the National Medical Insurance Bureau and the Health and Health Commission issued a "dual-channel" policy in time, and the negotiated drugs were guaranteed through the "dual-channel" of designated retail pharmacies and designated medical institutions. At present, the list of two batches of negotiating drug allocation institutions has been published, and 19 kinds and 97 kinds of negotiating drugs in the two batches of lists are allocated in 5,572 and 84,000 designated medical institutions respectively. The National Medical Insurance Bureau has opened a public inquiry channel on the National Medical Insurance Service Platform (APP), and the list of institutions equipped with negotiated drugs will be dynamically updated.
Recently, the "2021 National Medical Insurance Drug List Adjustment Work Plan" was published, marking the official start of a new round of national medical insurance drug list adjustment. The negotiation access stage will be in 9-mdash; It will be held in October. At that time, another batch of new drugs will enter the catalogue through negotiation, and the benefits of patients will continue to expand, further reducing the burden on patients.
